All students, 11 years of age, entering the 6th grade in the fall MUST have a dose of Tdap vaccine. If the student has received the Tdap vaccine, an updated Blue Slip/Immunization Record is required to be on file at the school.  This immunization is required at age 11 years old or older if entering the 6th grade.  There is a Tdap vaccine available for 10 year olds also, IF entering 6th grade.  The 6th grade students that DO NOT have an updated Blue Slip will NOT be allowed to attend class the 1st day of the school year.

All medication, prescription and non-prescription, will need to be picked up at the end of the year.  The medication will not be given to the student to bring home.  Any medication left at the school will be disposed of properly the last day of school.  Medication must be picked up by the end of the last school day.
